Introduction: Despite efforts to institutionalize public health policies, health judicialization still persists due to shortcomings in medication provision. Objective: To analyze the judicialization of medications in Altamira, Pará, from 2018 to 2022. Methodology: A cross-sectional, descriptive, quantitative, qualitative study based on data obtained from Judicial Processes and the Health Judicialization Map using the Anatomical, Therapeutical, Chemistry system. Results: Out of 43 processes, seven were requesting medications, with cardiovascular anatomical system medications being prevalent, such as angiotensin II receptor blocker, low-ceiling diuretic, and simple lipid-modifying agent. Three demands for monoclonal antibodies were evidenced for antineoplastics and immunomodulators. Conclusion: In Altamira, Pará, judicial demand for medications highlights the prescription of medications not present in official lists of the Unified Health System; as well as being an indicator of the need to qualify pharmaceutical care aiming for efficient management to avoid judicialization. Challenges persist in the implementation of constitutional rights, including access to medications.
